NEW CLIENTS

New Clients booking a Natural Health Consultation need to request and complete a New Client Form through the contact form below. It will be emailed to you and must be submitted at least 48 hours BEFORE your appointment. If forms are not received at least 48 hours before your appointment it will be rescheduled.
